What clinic management software costs in India, and what you actually need

What you are actually paying for

Clinic software in India is sold three ways, and the sticker price of each hides a different set of costs.

• Cloud subscription, per clinic. Typically ₹1,000–₹6,000 a month depending on which modules you switch on. Updates, backups and support are included because the vendor runs the servers.
• Cloud subscription, per user. Looks cheaper at ₹400–₹900 a head until you count the front desk, three doctors, a pharmacist and a lab technician. A six-person clinic pays more here than on most per-clinic plans, and the bill grows every time you hire.
• One-time on-premise licence. ₹25,000–₹1,50,000, plus an annual maintenance contract of 15–20% if you want updates, plus a machine to run it on, plus somebody to take backups. It is not cheaper; it is front-loaded and then billed differently.

The costs that are not in the price

Five charges account for most of the gap between what a clinic expects to pay and what it ends up paying.

1. Setup and training fees of ₹5,000–₹25,000, sometimes described as "implementation". Ask whether it is refundable if you leave in the first month.
2. Per-patient or per-visit pricing. Fine at 200 visits a month, punitive at 2,000. It also means a good month costs you more, which is a strange thing for software to do.
3. SMS and WhatsApp credits billed separately, often at a markup on the ₹0.10–₹0.85 per message the gateway actually charges.
4. Data export fees. Some vendors charge to give you your own records back. Establish before signing that export is free, self-serve and in a format something else can read.
5. Module unlocks. Pharmacy and labs are frequently sold as add-ons rather than plan tiers. Price the configuration you will actually run, not the base plan.

What a small practice genuinely needs

A single-doctor clinic needs four things and can safely ignore the rest of the feature list for a year:

• A patient record with history, allergies and past prescriptions.
• An appointment diary the front desk and the doctor both see.
• Prescriptions that print on your letterhead.
• Billing that produces a compliant invoice and tracks what is unpaid.

Add a pharmacy module only if you dispense, and labs only if you have a lab on site. Buying them "for later" means paying for two years of a module you will configure the week you finally need it.

Questions worth asking before you sign

• Can I export every patient, visit, invoice and stock movement myself, today, at no charge?
• Is the price per clinic or per user, and what happens when I hire?
• Are pharmacy and labs included in this tier or billed on top?
• What happens to my records if I stop paying — are they readable, and for how long?
• Where is the data stored, and who else can read it?
• Is there a contract minimum, or can I cancel from inside the product?

A reasonable budget

For a clinic of one or two doctors with a front desk, ₹1,000–₹2,500 a month buys everything that matters in 2026. Add roughly ₹1,500–₹3,500 if you run a dispensing pharmacy, and similar again for an in-house lab. If a quote is far above that, the difference is usually per-user pricing or a module unlock rather than a better product.
